Provides health and social services to those who experience barriers to care including poverty, homelessness, language or culture, and complex and/or chronic health conditions, mental health and addictions. Including support for transgendered individuals, youth, diabetes and chronic diseases, HIV and Hepatitis C, immigrant and ethno-cultural communities, mental health care, poverty and homelessness, primary health care, home visiting and drop-in support groups for seniors. Also offers needle exchange and Naloxone kits and training for individuals at risk of drug overdose.
